Managers and Supervisors should report all employee or volunteer work-related injuries or illnesses to the Human Resources Department within one (1) working day of notice or knowledge. Please don't wait until the Report of Incident, Accident, and Injury Form has been completed to report the injury; it's better to call Human Resources as soon as possible at (909) 798-7514. Some injuries require notification to governmental agencies and a delay could put the City at risk.

A Report of Incident, Accident, and Injury Form must be filled out for every injured employee, and volunteer.

All employees and volunteers are encouraged to report any work-related accidents and/or near miss incidents when they occur. Early reporting assists in identifying potential safety issues and in determining eligibility for benefits.

If an employee is injured and requires attention by medical professionals, a Manager or Supervisor (if uninjured at the time of the incident), is requested to take the employee to receive medical care at Fox Occupational Health Center.
