What is a Flexible Spending Account?
Flexible Spending Accounts allow you to set aside money-tax free-then use that money when you have certain everyday expenses, such as costs related to child care and health care. You must enroll (or re-enroll) in the flexible spending accounts annually to participate.
- Employees who would like to continue their FSA plan into 2025 will need to re-enroll via the PayPro Portal at https://pagroup.lh1ondemand.com/login.aspx.
- Employees who would like to enroll into an FSA plan will need enroll via the PayPro Portal at https://pagroup.lh1ondemand.com/login.aspx.
What do I need to know about Vision Insurance?
Vision insurance is only available for sworn safety and executive staff. All other employees who do not fall under the above categories receive up to $300 vision reimbursement each fiscal year.
For new enrollments or changes to your current plan, you will need to complete the Principal Change Form for Dental and Vision.
What do I need to know about Dental Insurance?
City employees can enroll in either plans: Principal (PPO) or Liberty Dental (HMO).
A PPO dental plan, also known as a Preferred Provider Organization dental plan, is a type of dental insurance plan where you have the flexibility to choose any dentist of specialist, whether they are in-network or out-of-network.
For new enrollments or changes to your current plan, you will need to complete the Principal Change Form for Dental and Vision.
A HMO dental plan, also known as a Health Maintenance Organization dental plan, is a type of dental insurance plan where it requires members to choose a primary care dentist (PCD) from a network of participating dentists.
For new enrollments or changes to your current plan, you will need to complete the Liberty Dental Enrollment Form.
What do I need to know about Voluntary Life Insurance?
Employees can elect or make changes to Voluntary Life Insurance during this open enrollment. Please note that if you are submitting a change or new election during open enrollment, you and/or your spouse must complete the Principal Voluntary Life Health Statement.

What do I need to know about Reliance Insurance?
Disability income protection insurance provides a benefit for “long term” disability resulting form a covered injury or sickness.
For new enrollments or changes to your current plan, you will need to complete the Reliance Long Term Disability (LTD) Enrollment Form.

What do I need to know about Colonial Life Insurance?
Employees wishing to enroll in or inquire about Colonial Life are required to contact a Colonial Representative at 855-697-6876. No appointment is necessary, and you may contact the representative at any time during this open enrollment.
